Hackensack Meridian Health and healthcare technology company Lumeris are expanding their value-based care partnership to include traditional Medicare beneficiaries, a move expected to strengthen care coordination for seniors across the greater Meadowlands region and Northern Jersey.
The expansion brings traditional Medicare patients into the Hackensack Meridian Health Accountable Care Organization under the federal Medicare Shared Savings Program, extending a collaboration that already includes commercial and Medicare Advantage populations.
For the Meadowlands region — home to several Hackensack Meridian Health hospitals, outpatient facilities and physician practices — the initiative is designed to improve access to coordinated, high-quality care while reducing unnecessary hospital use and healthcare costs.
“Our physicians are committed to delivering connected, high-quality care for seniors,” Patrick Young, president of population health at Hackensack Meridian Health, said in a press release. “By expanding our work with Lumeris, we will leverage our strong foundation and proven workflows to improve outcomes and coordination for Medicare beneficiaries while supporting our clinicians with the tools and insights they need at the point of care.”
Meadowlands and AI Central to the Program
The partnership will serve nearly 46,200 Medicare beneficiaries statewide through Hackensack Meridian Health Partners, the system’s clinically integrated network of primary care providers. Many of those physicians practice in communities throughout Bergen and Hudson counties, making the Meadowlands a key part of the program’s footprint.
Lumeris will support the ACO with its AI-powered Tom platform, embedded in clinical workflows to help care teams manage patient populations, track performance, and identify proactive next steps in patient care. The technology is designed to support preventive and long-term care for seniors while reducing the administrative burden on physicians.
Proof is in Preliminary Results, Improved Outcomes

Hackensack Meridian Health’s ACO has already demonstrated strong results under value-based care models. In 2024, the organization earned an 87.27% quality score from the Centers for Medicare & Medicaid Services and generated more than $40 million in total healthcare savings. More than $18 million of those savings was returned to the ACO to reinvest in patient-focused initiatives.

The organizations said the expanded collaboration will continue to focus on improving quality, patient experience, and total cost of care by aligning providers around shared performance goals and coordinated care processes — a model that is increasingly important as New Jersey’s population ages.
